Joseph Frazier served in the 15th Ohio Infantry in Companies B and G.
Joseph Frazier, a private of 1st Lt. Lucius O. Doolittle's Company (G) of the 15th Regiment of the United States was enlisted by Captain Merdith [sic] at Columbus, Ohio on the 7th day of September 1862 to serve 3 years.
He was born in Belmont County in the state of Ohio.
He is 35 years of age
He is 5 foot 11 inches high
Complexion is Sandy
Eyes are Hazle [sic]
Dark hair
Occupation when enlisted: Merchant
Soldier's Address: Town Martin's Ferry, Belmont County, Ohio.
A ball entered the front of his left arm and through it into his thorax and came out through the lower part of the scapula, fracturing it.
The doctor also states it is his opinion that a rib was fractured and the lung was either penetrated or injured to the extent of producing extensive adhesions of the pleura to the wall of the chest.
The wound was contracted at the battle of the Kenisaw Mountains, Georgia on the 21st of June 1864.
Joseph Frazier, formerly a private of Company G in the 15th Regiment of the Ohio Volunteers in the War of 1861 was paid at Des Moines Agent at the rate of $10.00 per month on account of a gunshot wound of the left arm and left shoulder.

The State of Ohio, Belmont County.
I certify that on the 21st day of April 1857, Mr. Joseph Frazier and Miss Tabitha Spence, were by me legally joined in marriage. Signed B. Mitchell, V.D.M. Certified by James F. Tallman, Probate Judge, State of Ohio, Belmont County, dated 29th Day of October 1891.
"Joseph Frazier was admitted to the Nebraska Hospital for the Insane[,] September 13 1890 and remained there under our professional care until July 13th 1891. He was suffering during the whole time with Sub Acute mania with delusions and during his residence in the hospital was totally incapacitated for mental or physical labor. When admitted to the hospital there were several scars on his body which are belieived [sic] to be the result of gun shot wounds. He complined [sic] more or less during the whole time of his residence in the hospital of pain in the chest which he beleived [sic] was due to a bullet which had lodged there during the war of the rebellion. His height was 5 ft 9 1/2 inches and his weight at the time of admission was 172 obs. No physical trouble of any serious nature was discovered until about a month before his discharge when he began to complain of dyspnoea [sic] while in the recumbent position and had weak and irregular action of the heart. He also complained of unusually severe pain in the chest. These symptoms were soon followed by oedema of the fee and legs and dropsical effusion into the peritoneal cavity. While in this condition he was removed from the hospital by his friends. They further declare that they have no interest either direct or indirect in the prosecution of any claim for pension in connection with this case."

"Mrs. Tabitha Frazier is still the widow of Joseph Frazier, and is without other means of suppport than her own daily labor, and she is an invalid.
She owns a home on west side of Fairmont, the assessed value is $200 (two hundred dollars), lot is not numbered.
We live next dore [sic] neighbors one on either side; she could not have bean [sic] without our notice and we know of our own personal knowledge, that the claimant is not possessed of any property other than that above discribed [sic] and that she has not remarried since the soldiers death."

On this sixteenth day of September, A.D. one thousand eight hundrend and ninety one personally appeared before me, a Notary Public within the County and State aforesaid Tabitha Frazier aged 57 years, a resident of Fairmont, Fillmore County, Nebraska, who, being duly sworn according to law, makes the following declaration in order to obtain the pension provided by act of Congress granting pensions to widows, approved June 27, 1890, to wit:
That she is the widow of Joseph Frazier who served during the late War of the Rebellion under the name of Joseph Frazier from the seventh day of September, A.D. 1862 to the 24th day of September A.D. 1864, as a Private in Co. G. 15th REg. Ohio Vol. Inf. and who was honorably discharged from the service at Columbus, Ohio Sept. 24th, 1864 and who died of heart disease with dropsey on the thirteenth day of August A.D. 1891 at Fairmont, Nebraska that she is without other means of support than her daily labor; that she was married under the name of Tabitha Spence to said Joseph Frazier on the twenty-first day of April A.D. 1857 by Rev. B. Mitshell [sic] at Mount Pleasant, Ohio, there being not legal barrier to such marriage; that neither she nor her husband had been previously married.
--- That she has not in any manner been engaged in, aided or abetted the rebellion in the United States; that said Joseph Frazier, at the time of his decease was receiving a pension under the certificate no. 37,784.
She hereby appoints, with full power of substitution and revocation, George E. Lemon, of Washington, D.C., her true and lawful Attorney, to prosecute this claim. That her Post-office address is Fairmont, Fillmore County, Nebraska.
